We provide comprehensive, multidisciplinary in-home health care services to promote, maintain and restore health and to minimize the effects of illness and disability. AZHomeHealth’s team of knowledgeable professionals is dedicated to excellent, compassionate care for patients, clients and their families.
AZHomeHealth is a home health agency who prides themselves on giving our patients the highest quality of home care. Our home health care professionals include RN’s, LPN’s, CNA’s, and home health aides, who provide care to those desiring to remain living at home as well as those who are in need of wound care and skilled nursing.
We work closely with both the patient and the physician to determine the exact level of home health care needed. The care provided may be temporary, consisting of a few days a week for a few short weeks, or it may be essential to be seen daily for a couple of months. Whatever your needs, we will provide the care that is necessary to restore optimum health.
AZHomeHealth believes that home care should be more than just nursing attention. We believe that in order to provide the highest quality of care, we must treat our patients like our own family. We select only nursing staff who have strong backgrounds in providing reliable care. We have ongoing training for our nurses and aides to ensure they see patients as more than just medical situations; our patients become our first priority and are as important to us as our own family.
AZHomeHealth works with patients and families to:
- Ease the transition from a hospital or other health care facility to a patient’s home
- Return patients to maximum health
- Provide counseling and coordination of community services to meet patients’ needs

- Administer quality health care services within the home when caregivers are unable or unavailable to provide those services
- Instruct them in the fundamentals of health, hygiene and safety as well as the prevention, control, and treatment of disease.
Home Care Services
Home care includes a broad spectrum of in home health care and social services, including nursing, therapy treatments and/or assistance with the activities of daily living. Increasingly, the home is the appropriate place to deliver these services for those who wish to remain independent but have the need for care that is beyond the scope and family’s and friends’ abilities. Home care providers deliver highly skilled services in conjunction with modern technology that enables the elderly, chronically ill, disabled and recuperating patient to receive sophisticated medical treatment in the comfort and privacy. Being in their own surroundings can be more soothing, and, therefore, more conducive to recovery.
Skilled nursing:
Registered Nurses (RN’s) and Licensed Practical Nurses (LPN’s) provide the skilled care designed to meet a medical need. Treatment is delivered in accordance with a physician’s orders and as defined in a care plan. Nurses often coordinate care between the different disciplines The following are examples of nursing care provided in the home:
- Medication management
- I.V. therapy
- Wound/Ostomy care
- Post-surgical care
- Understanding a new disease process or diagnosis
- Teaching a patient or family member ways to promote the patient’s self care and the highest possible level of independence
- Promote recovery
- Bowel and bladder management
Physical Therapists specialize in treatments designed to improve the mobility and physical strength of patients, physical therapy consists primarily of physical exercise. Therapies used help alleviate pain and restore muscle strength. Physical therapists also teach special techniques to improve mobility. Physical therapy is delivered in accordance with physician’s orders and as defined in a plan of care. Home Health Aides assist patients in the completion of daily living activities and personal care. Some are specially trained to deliver more complex services under the direction of a registered nurse.
Eligibility
Home care is available for anyone. Medicare, commercial insurance and private pay arrangements are all possible payment sources for home health care services. However, third party payers have specific criteria that must be met for coverage of services.
